INSTRUCTIONS FOR REGISTERING AND PAYING FOR TRAINING CLASSES AND MATERIALS

Class Registration: Read the descriptions for our training and examination options. Then register for the class or exam you want using the sign-up form in "Step 2" below. When the sign-up form is completed, click "Send" at the bottom of the form. Another registration option is to copy the fax form. Complete and fax the form to: 908-393-2957 or scan and send to: info@sqfguy.com.
Payment: Once registration process is completed, click here to go to our e-commerce website. You may also pay later by returning to the Training Tab on our main website, and clicking on the "Check out and Pay Tab". Payment must be received within 7 days to ensure a seat in the class. We accept MasterCard and Visa but not American Express. We also accept payment by check. Once a payment clears our bank, we will send confirmation.

INTRODUCTION TO FOOD SAFETY AND GMPs.

Location: Hyatt House, Bridgewater, NJ 908-725-0800, Group rate $149 per night includes breakfast, beer, wine, and light dinner Enrollment Fee: $250 includes lunch, course slides, sample exercises, quizzes and certificate. This class meets the requirements for employee training required under 21CFR 110. It also fulfills the requirements of the Food Safety Modernization Act, USDA requirements, and the SQF Institute for SQF Practitioners.

- Attendance at all sessions, completion of class booklet and passing exam is mandatory for Certificate of Achievement.
HAZARD ANALYSIS & CRITICAL CONTROL POINT WORKSHOP (HACCP)
Dates: March 20 & 21, 2012 8:15 - 4:30 pm
April 24 & 25, 2012 8:15 - 4:30 pm
Location: Hyatt House, Bridgewater, NJ 908-725-0800, Group rate $149 per night includes breakfast, beer, wine, and light dinner Enrollment Fee: $475 includes lunch, course slides, sample exercises, quizzes and certificate recongized by the International HACCP Alliance as meeting the requirements of the Food Safety Modernization Act, USDA requirements, and the SQF Institute HACCP requirement for SQF Practitioners. (Enroll in the SQF class as well, and both rates are discounted to $1030)

IMPLEMENTING SQF 2000 SYSTEMS (Practitioner) Workshop

- Attendance at all sessions & completion of class booklet is mandatory for receiving a Certificate of Attendance. This course is an SQF Institute approved class and trainer.
- Completion of HACCP training is a requirement to become a Practitioner.

FOOD SAFETY TRAINING MANUAL FOR FOOD PLANT EMPLOYEES.
Introductory food safety training manuals available in English and Spanish, for production floor employees and supervisory staff who need to brush up in the fundamentals of food safety and HACCP. Texts are easy-to-read and come with a self-test on such topics as GMPs, pest control, HACCP, foreign matter control and other important issues that are expected to be covered during the audit process by FDA or 3rd party inspectors. Recommended reading prior to attending our Introductory Food Safety and GMP or HACCP workshops.

You may either (1) Fill out the form below and hit "send" or (2) copy the "Fax Registration Form" on our Training Tab, complete it, and fax it back to the number on the form or scan and e-mail it to info@sqfguy.com.

Registration Step No. 3: Complete Payment
Proceed to our e-commerce site in order to check-out and complete payment. If you elect to use a credit card, you must enter the billing address for the card in the block titled "Bill To". Registration confirmation will be sent to you by e-mail once payment is received.

Cancellation policy: Full refund if canceled 14 days prior to start of class. Cancellations 14 - 10 days prior to class start will be charged 50 % of the class registration fee. No refund if you cancel 10 days or less prior to class start. Cancellations must be made in writing. If you cancel prior to 10 days, you may attend the next class by paying an administrative fee of $125.
